Our employees make all the difference in our success!Job Profile SummaryRole:Under RN supervision, the LPN is responsible for providing nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.Qualifications:A high school diploma or G.E.D. or an equivalent combination of education and experienceCurrent LPN licensure in the state of employmentIV certification required at time of hire beginning November 10, 2024. Employees in this job prior to November 10, 2024 will be required to obtain certification by June 10, 2025.Active BLS for healthcare professionals from the American Heart Association or Red Cross.For current Hope Employees: BLS should be obtained by January 1, 2025For current Capital Caring employees: BLS should be obtained by November 1, 2024Mobile Driver - Valid driver's license and automobile insurance per Company policyReliable transportation to meet visit scheduleIf working in an inpatient unit certificate of completion of 30 hour Infusion Therapy Course and in-house 8-hour Infusion Class preferredAbility to communicate (written and verbal) in EnglishFlexible schedule to meet the needs of patientsAbility to use equipment with visual and auditory mechanismsAbility to manage the emotional stress of working with and caring for terminally ill patients and their familiesAbility to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodationCan work within their scope or practice per state regulationsCompetencies:* Satisfactorily complete competency requirements for this position.Responsibilities of all employees:Represent the Company professionally at all times through care delivered and/or services provided to all clients.Comply with all State, federal and local government regulations, maintaining a strong position against fraud and abuse.Comply with Company policies, procedures and standard practices.Observe the Company's health, safety and security practices.Maintain the confidentiality of patients, families, colleagues and other sensitive situations within the Company.Use resources in a fiscally responsible manner.Promote the Company through participation in community and professional organizations.Participate proactively in improving performance at the organizational, departmental and individual levels.Improve own professional knowledge and skill level.Advance electronic media skills.Support Company research and educational activities.Share expertise with co-workers both formally and informally.Participate in Quality Assessment and Performance Improvement activities as appropriate for the position.Job Responsibilities:Under the direction of an RN, provides nursing tasks to patients and families such as continuous care, dressing changes, catheter changes as identified in the plan of care developed by the Interdisciplinary Group (IDG).Demonstrates clinical skills regarding application of Company policies and procedures such as: medication administration, symptom management, and patient/family relationships.Observes/monitors patients for alterations in body systems, pain and symptoms and reports to RN.Observes and reports signs of adverse reactions and responses to treatments and reports to RN.Reports findings to physicians, APRNs and physician's assistants following collaboration with RN.Performs venipuncture for blood specimens.Performs Central Venous Line (CVL) site observation/inspection; venous access device dressing, cap change and flushing venous access device; IV medication and fluid administration, IV pushes (per procedure) and venipuncture. May not make any changes to infusion pump including, but not limited to, rate changes and IV bag changes.Documents per policy and procedure in medical record.During times of emergencies (i.e. Hurricanes, etc.), the LPN may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergencyPerforms other duties as assigned.Some locations may require:* Cross training to Hospice House positions of equal licensure/certification and scope when census calls for flexible staffing.Compensation Pay Range:$17.21 - $25.81This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.All Chapters Health System employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse to verify eligibility after a conditional offer of employment is made as well as ongoing eligibility.
