Clinical Social Worker - Boca

Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and patient-focused Clinical Social Worker to join our team.This full-time position works Monday through Friday 8 a.m. - 5 p.m and one weekend every 6 weeks. This role covers clinics in the Boca area. In this important role, you will provide essential psychosocial assessments and interventions that help patients and families navigate end-of-life care-directly enhancing the quality, dignity, and comfort of their hospice experience.Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. Masters preferred.One year of experience as a Social Worker.
