Dunkirk Rehabilitation and Nursing Center - Qualifications/Experience:Must possess a current unencumbered NYS license as a Registered Professional Occupational Therapist, Bachelor's degree or higher.Graduate of an accredited school of occupational therapy or international equivalentGeriatric experience preferredA current unrestricted driver's license and dependable transportationAble to use necessary computer applicationsAbility to professionally communicate with internal and external resourcesHigh level of motivation and ability to take initiative appropriatelyAbility to function independently as well as part of a teamAbility to function effectively in stressful and changing situationsAbility to positively give and receive oral and written directionJob Summary:The primary purpose of this position is to provide Occupational Therapy services to include: Evaluations/Screens for Occupational Therapy services; plans appropriate Occupational Therapy services per therapy plans of care; provides Occupational Therapy treatment in accordance with accepted professional standards; communicates with other disciplinary team members to ensure resident needs are met and participates in general functioning of the rehabilitation therapy department.Essential Job Functions:Every effort has been made to make your job description as complete as possible. However, it does not in any way state or imply that these are the only duties you will be required to perform.Demonstrates working knowledge of skilled restorative therapy and maintenance programsScreens residents for rehabilitation therapy needsCompletes Occupational Therapy evaluations upon physician orderEstablishes Occupational Therapy Plan of Care for treatments per physician orderProvides Occupational Therapy plan of treatments and modalitiesModifies treatment program level, intensity and techniques based on the resident's response to specific treatment planDocuments resident progress toward occupational therapy goalsDischarges residents from Occupational Therapy services when appropriateMakes appropriate recommendations for nursing regarding mobility, positioning, ROM and Self-care/ADL tasksCompletes Daily Activity Schedule, progress notes and re-certifications per facility policy.Supervises Certified Occupational Therapy Assistants and unlicensed assistive personnelProvides resident, staff and family education and in-servicingDemonstrates working knowledge of Medicare Part A PPS case managementComplies with Federal and State regulations related to rehabilitation therapyCompletes all documentation to comply with professional standard and regulatory requirementsStrives to meet productivity / efficiency goals and self-initiates corrective action plan for non-attainmentUtilizes the clinical software applications per policies and proceduresCompletes care plans and MDS as directed, per facility policyAssists in ordering necessary equipment for the facility and residentsParticipates in the discharge planning process with the interdisciplinary teamParticipates in facility meetings and committees as assignedSubmits required reports and documentation in accordance with established acceptable time framesParticipates in internal audits as directedPossess a strong knowledge of Medicare, Medicaid and other payment sources and their responsibility in the SNF settingUtilizes proper body mechanics, techniques and safety while completing job duties and responsibilities.Maintains current professional knowledge of practice of Occupational Therapy and practices within acceptable professional standardsComplies with all facility policies and proceduresMaintains confidentiality of facility and resident informationRespects all residents all resident rightsReports all instances of potential resident abuse, neglect, and misappropriation of resident property immediatelyFollows sanitation and infection control policies and proceduresKeeps Facility Rehabilitation manager informed and updated appropriatelyRecommends corrective action plan for all identified problemsMaintains a positive working relationship between the rehabilitation department and other departmentsMaintains high standards of professional conductPhysical Requirements:In good physical healthAble to push wheelchairs/geri-chairs with residentsAble to push supplies on carts weighing up to 50 poundsAble to walk, bend, stoop, kneel, and climb/descend stairs throughout shiftAble to reach above and below shoulder heightAble to lift and carry supplies and equipment weighing approximately 25 poundsJOB DESCRIPTION REVIEWI have read this job description and fully understand the requirements set forth therein.
