Social Worker Supervisor

Highland Care Center - Highland Care Center, a 320 bed Skilled Nursing Facility in Queens, is seeking a Full Time Social Worker. The candidate shoud have experience in a long- term care/sub-acute setting.Duties include the following:Interview resident/families for compilation of a social history and completion of a pyschosocial assessment.Develop and maintain care plans based on individual psychosocial needs of residents.Facilitate and participate in the interdisciplinary care plan meetings.Strengthen and facilitate communications among residents, families, and staff.Act as a discharge planning coordinator to ensure safe discharge to the community.Provide supportive counseling and assistance as clinically indicated.Requirements:B.A. in Social Work or related field.MDS experience preferredNYS Screen certification preferredExperience with EMR (Point Click Care) is a plusExcellent writtnen and communication skills.
