Licensed Mom and Baby Care Manager

Licensed Mom and Baby Care Manager, a full-time salaried position, is responsible for collaborating with an inter-disciplinary care team to improve member health outcomes through care coordination, assessments, and the development of individualized care plans. Key Responsibilities Engage members and their support systems through assessments and care planning Facilitate inter-disciplinary care team meetings and coordinate communication among team members Monitor and evaluate member progress and satisfaction with care plans Required Qualifications, Training, and Education Bachelor's degree in a health care field with three years of relevant experience required Advanced degree associated with clinical licensure preferred Minimum of three years of experience in social work, counseling, or health care profession required Three years of Medicaid and/or Medicare managed care experience preferred Current unrestricted clinical license in state of practice as a Social Worker or Professional Clinical Counselor required
