Job Description We currently have an opportunity for a Bi-lingual Chinese-speaking Registered Nurse in Brooklyn to make visits to patients' homes. Key Functions:Perform initial assessment of clients for admission to home care, reassessments, and post-hospital discharge visitsSupervise and instruct PCA/HHAsReview medicationDevelop aides care plan, and MD orders in HHA ExchangeCollaborate with medical providers, and other members of the interdisciplinary teamWork closely with coordination departmentPerforms patient/family educationDischarge planningQualifications:Current NYS RN License & Registration requiredBilingual English-Chinese requiredHome care/experience with adult/geriatric population preferredMust be willing to travel to clients homesAbility to use computerized systemsExcellent verbal and written communication skillsCustomer service skills are essentialEOE/M/F/D/VJob Type: Per DiemPay: $100.00 -$130 per visitSchedule: variesExpected hours: variesExperience:Home care experience preferredHHA exchange: preferredShift availability:Day Shift (Required)Work Location: In person
