Citadel Care Centers - The Bronx Gardens Rehabilitation & Nursing Center is currently seeking a Full -Time LPN's for day (7am-3pm), evening (3pm-11pm) night (11pm-7am) AND Weekend shifts to join our dynamic organization.OVERVIEW:Under the leadership of the Registered Nurse, the LPN will be providing coordinated care to residents in a skilled nursing facility.RESPONSIBILITIES:· Provide direct care responsibilities to patients· Supervise CNA's· Administer medications and treatments timely and according to policy· Receive, transcribe, and carry out physician orders· Follow established procedure for charting· Complete patient assessments, coordination of care· Identify and reports changes in condition· Follows environmental health & safety and sanitation standards and procedures· Oversees clinical and non-clinical acute operations and continuum of careREQUIREMENTS:· Valid LPN License in New York· CPR/BLS certified· 1 + year nursing experience in a hospital, nursing home or other healthcare setting
