Internal Medicine - Medical Director opening in VA - Located in Norfolk, VA - Full-time, permanent position - BE/BC in IM or FM - At least three years of experience as a practicing physician.About the Role:Facility is seeking a Family Medicine/Internal Medicine Physician to join and lead our Sentara PACE (Program for All-inclusive Care for the Elderly) team in Norfolk, VA. This is a full-time opportunity to manage an interdisciplinary team focused on quality and clinical outcomes for a small panel of 150500 elderly patients. This is an innovative program providing comprehensive healthcare and supportive services to frail seniors who prefer to remain in their homes and community. It offers an alternative to nursing facilities, with care coordination that includes medical, rehabilitative, in-home services, and transportation support.Position Highlights: - Innovative Practice Models - Clinic Duties: Primary care for patients aged 55+ in a low-volume clinic (610 patients/day). - Home Visits: Occasional home or assisted living facility visits as needed. - Schedule: 8-hour workdays (8:00 AM4:00 PM), rotating call schedule every three weeks (Monday 5:00 PMMonday 8:00 AM). - Collaborative Environment: Interdisciplinary team meetings (12 hours most days). - Support: On-site APP for daily patient management and re-certifications. - Focus: Value-based care, with emphasis on patient outcomes and quality.Benefits Package: - Competitive salary with bonus opportunities. - Comprehensive benefits, including retirement plans. - $10,000 Student Loan Reimbursement Program. - Malpractice and Tail Coverage. - Fully-integrated EHR system (Epic).
