Responsibilities: - Assessing the patient’s physical, communication, and cognitive skills to develop a treatment plan -Assessing the home, and work environments of the patient - Implement therapeutic exercise programs - Reviewing the patient’s progress and adapting the treatment accordingly - Educate patients on proper use of equipment and exercises - Performing administrative tasks such as keeping records, writing reports and making telephone calls - Maintain patient records accurately - Collaborate with healthcare professionals to ensure comprehensive patient care Experience: - Experience or interest in working with geriatric patients - Strong patient care skills - Familiarity with post hospital interventions and post-acute care environments - Knowledge of human anatomy and physiology - Understanding of medical terminology - Experience in Home Health Physical Therapy Services.
