Director Case Management

Reviews medical records and identifies cases needing follow-up. Collects and organizes data for committee review. Uses sound judgment in case referrals. Leads and promotes effective case management practices. Evaluates program performance and implements improvements. Collaborates professionally with physicians, staff, and administration. Guides hospital-wide initiatives in resource management and patient flow. Oversees assigned projects and represents the department as needed. Maintains confidentiality in all quality review activities. Educates staff on case management and quality concepts.
