Staff RN/Case Manager - Wound Healing Center - FT Weekdays
RN Case Manager The RN Case Manager performs the primary function of a RN in assessing, planning, implementing and evaluating the care of patients in the Wound Healing Center. The position is responsible for meeting the established WHC Standards of Nursing Practice in the management of all assigned patients. This position assists physicians with the development and implementation of wound treatment plans and participates in performance improvement review activities. The RN Case Manager acts within the guidelines of federal and state laws, Joint Commission Standards and professional and ethical practice. In the execution of job duties, it is the universal expectation that all tasks are performed with a patient centered focus, while also seeking opportunities to continually improve core processes. Incumbent will be scheduled based on operational need, which may include but is not limited to: holidays, extended shifts, night and/or weekend shifts, standby and/or on-call. Reports to: Director, Wound Care Center Essential Functions Assess, stage, and measure wounds accurately and in accordance with policy. Assist physician with wound therapy. Collaborate with physician to develop a plan of care based on assessment data. Develop and implement a teaching plan. Formulate desired outcomes that are specific and measurable within a certain time frame and consistent with other health provider's expectations. Monitors patient and plan of care and makes adjustments as necessary. Communicates and documents changes. Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances. Patient/family care needs and/or problems are promptly identified and addressed. Patients are regularly assessed and reassessed. Patient responses to nursing and MD interventions and treatments are evaluated and documented in the ongoing assessment data. Functions successfully in all emergency situations. Adapts to change and handles emergencies consistent with policy and in a manner that instills confidence in others through a smooth operation. Establishes individualized nursing diagnoses based on assessment data and formulates care plan. Collaborates with patient and family members to address care needs and provide patient centered care. Adjusts plan of care appropriately. Knowledgeable and incorporates best practices. Complete the 4 week reviews in a timely manner and ensure all recommendations to the plan are activated in the physician orders. Accurately complete the super-bill and ensure that all orders and charges are initiated accurately. Minimum Education Bachelor's degree preferred required Required Skills Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Certifications: Basic Life Support (BLS) Licenses: Current Virginia State RN License Minimum Work Experience Required: Minimum of 2 years of clinical experience. Preferred: Wound care experience.
