Care Coordinator

Job Title: Virtual Care Coordinator/Telehealth Care CoordinatorLocation: New York, NY 10017Shift: 5 Days/Week | All Shifts Available (7 AM – 3 PM and 11 PM – 7 AM)Contract Duration: 3 Months with strong possibility of further extensionPay Range: $24-29/hr.Key Responsibilities:Support telehealth and remote patient monitoring programsMonitor patients remotely for safety and clinical needsCoordinate virtual care workflows and telehealth servicesTrack quality metrics and identify workflow improvementsTroubleshoot telehealth platform issues with staff and vendorsTrain staff on telehealth workflows and documentationMonitor Epic telehealth consult orders and scheduled visitsDocument patient observations and escalate safety concernsCollaborate with clinical teams to improve patient outcomesRequired Qualifications:Associate’s Degree required; Bachelor’s in health-related field preferred2+ years of telehealth or remote monitoring experience required3+ years of direct patient care experience preferredExperience with EMR systems and EpicStrong communication, organization, and multitasking skillsKnowledge of HIPAA compliance and telehealth workflowsExperience in quality assurance and policy developmentPreferred Skills:Remote patient monitoring experienceTelehealth workflow developmentData analysis and reportingStrong troubleshooting and problem-solving abilities“Pride Health offers eligible employee’s comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance, and employee discounts with preferred vendors”.
