CCH Healthcare - MDS Coordinator (Float) – Full-TimeCCH Healthcare – Cincinnati, OH (Harrison Pavilion & Cedarview)Position SummaryCCH Healthcare is seeking a full-time MDS Coordinator (Float) to support two of our Cincinnati-area facilities: Harrison Pavilion and Cedarview.This role plays a critical part in ensuring accurate MDS assessments, regulatory compliance, and optimal reimbursement across both buildings. The ideal candidate is detail-oriented, organized, and experienced in skilled nursing operations, with the ability to adapt between facilities as needed.Key ResponsibilitiesComplete and oversee MDS assessments (OBRA, PPS, and Medicare/Managed Care)Ensure accuracy and timeliness of all assessments and submissionsCoordinate with interdisciplinary team (IDT) to capture accurate clinical dataMaintain compliance with federal and state regulationsMonitor case mix and identify opportunities for reimbursement optimizationConduct chart reviews and audits to ensure documentation supports codingAssist with care planning process and IDT meetingsProvide coverage and support between Harrison Pavilion and CedarviewCollaborate with facility leadership on quality measures and survey readinessQualificationsActive RN or LPN license in Ohio (required)MDS experience in a skilled nursing facility (required)Strong knowledge of RAI process and PDPMExperience with PointClickCare (PCC) preferredStrong organizational, analytical, and communication skillsAbility to manage priorities across multiple locationsSchedule & Work EnvironmentFull-time positionOn-site role supporting both:Harrison PavilionCedarviewSchedule may vary based on facility needsWhy Join CCH HealthcareStable leadership and supportive clinical teamsOpportunity to work across multiple facilities and broaden experienceCompetitive compensation packageGrowth opportunities within a multi-facility organizationHow to ApplySubmit your resume through Indeed to be considered.
