Job Description Registered Nurse (RN) – Home HealthJob Type: Permanent, Full-timeSchedule: Days, 8:30am - 5:00pmPosition SummaryWe are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality nursing care to patients in their homes. This role is responsible for assessing, planning, implementing, and evaluating individualized patient care plans while collaborating closely with physicians, caregivers, and interdisciplinary healthcare teams to ensure safe and effective outcomes.The ideal candidate is organized, clinically strong, and passionate about delivering patient-centered care in a home health setting.ResponsibilitiesProvide professional nursing care to patients in the home setting in accordance with physician orders, organizational policies, and regulatory standards.Conduct comprehensive nursing assessments, medication reconciliation, and development of individualized care plans.Monitor patient progress and update care plans based on changes in patient condition.Ensure visit frequency and services align with physician orders and insurance authorizations.Deliver and document patient and caregiver education using appropriate teaching strategies.Perform assigned nursing treatments and procedures according to established standards of care.Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care and resource referrals.Accurately document assessments, interventions, clinical decisions, and patient progress within required timelines.Communicate patient status updates and care plans during care transitions and staff handoffs.Identify and report patient care concerns to leadership and physicians in a timely manner.Supervise and support home health aide staff, including completion of required supervisory visits and documentation.Maintain productivity standards and complete assignments independently and efficiently.Stay current on nursing best practices, regulatory requirements, and clinical guidelines.Perform additional duties and projects as assigned.QualificationsRequiredAssociate's Degree in Nursing or Diploma in NursingActive New Jersey Registered Nurse (RN) LicenseAHA Basic Life Support (BLS) CertificationMinimum of:Six (6) months of RN experience, OROne (1) year of LPN experiencePrevious experience in home health or hospice preferredBasic computer proficiency and documentation skillsAbility to lift, push, and pull up to 50 lbs.Ability to stand and remain active throughout the workdayPreferredBachelor of Science in Nursing (BSN)IV CertificationSkills & CompetenciesStrong clinical assessment and critical thinking skillsExcellent communication and patient education abilitiesAbility to work independently and manage time effectivelyStrong documentation and organizational skillsCompassionate, patient-focused approach to careAbility to collaborate effectively within interdisciplinary teamsWork EnvironmentThis position requires travel to patient homes and the ability to adapt to various home care environments while maintaining a high standard of patient care and professionalism.
