In this unique and impactful role, you'll serve as the bridge between our Day Support and Group Supported Employment (GSE) programs, ensuring participants receiving services in both programs experience smooth, coordinated, person-centered support.If you enjoy problem-solving, collaboration, organization, and making a difference every day - this could be the perfect fit for you.About The RoleThe Dual PC will oversee coordination and integration of services for participants enrolled in both Day Support and Group Supported Employment (GSE) programs.This role ensures services are aligned, documentation is accurate, communication is seamless, and participants receive high-quality, uninterrupted support across programs.You'll collaborate closely with Program Managers, Day and Employment Support Specialists, participants, families or guardians, and support teams to help participants succeed both in the community and the workplace.What You'll DoParticipant & Program CoordinationCoordinate services for participants authorized in both Day Support and GSEAlign schedules across programs to prevent service gaps or overlapsSupport smooth transitions between Day Support activities and employment worksitesMaintain a consistent, accurate overview of participant servicesAttend intake meetings and assist with onboarding coordinationDocumentation & Billing IntegrityReview documentation for accuracy, completeness, and timelinessValidate participant time entries in ECHO's EHR and payroll systemsEnsure billing readiness and prevent duplication of servicesWrite quarterly reports within required timelinesReview participant factsheets and records for accuracySupport reconciliation between Day Support and GSE documentationISP & Authorization AlignmentMonitor WAMS authorizations and service utilizationParticipate in ISP/Person-Centered Planning processesEnsure ISP goals and supports are implemented consistently across programsAttendance & Utilization ManagementMonitor participant attendance, schedules, and service utilizationTrack absences, late arrivals, and scheduling changesIdentify and communicate concerns related to underutilization or inconsistenciesCross-Team CollaborationServe as a liaison between Day Support staff, GSE staff, and Program ManagersEnsure communication between teams is clear, timely, and consistentHelp staff stay informed on schedules, protocols, and participant needsStakeholder CommunicationCoordinate with families, support coordinators, residential providers, and interdisciplinary teamsMaintain consistent communication across all service areasCompliance & Quality AssuranceSupport compliance with Medicaid, DBHDS Licensure, and CARF standardsAssist with incident follow-up, investigations, and human rights documentation as neededHelp maintain the integrity and quality of participant recordsOperational SupportAssist with participant arrivals/departures and scheduling coordinationProvide cross-program support during staffing shortagesCross-train in both Day Support and GSE operationsSubstitute for ECHO drivers when neededWhat You BringBachelor's degree in Human Services + 1 year of experience supporting individuals with developmental disabilities OR Minimum of 5 years of direct experience supporting individuals with developmental disabilitiesIn Addition, You HaveStrong organizational and multitasking skillsExcellent written and verbal communication skillsAttention to detail and professional judgmentUnderstanding of clinical risk and person-centered supportsExperience with EHR systemsAbility to collaborate effectively across teamsFlexibility and reliability in a fast-paced environmentValid driver's license with satisfactory driving recordKnowledge of Person-Centered Plans (PCPs) and service authorizationsAbout ECHOFor 50 years, ECHO has empowered adults with disabilities to achieve their highest level of personal, social, and economic success.
