Description:Provides skilled occupational therapy service on an intermittent basis to consumers in their homes in accordance with the physician's ordersRESPONSIBILITIESAssists the physician in evaluating the consumer's level of function by applying diagnostic and prognostic proceduresConducts initial and ongoing comprehensive assessments of the consumer's needsEvaluates the consumer's significant other(s), and the home situation to determine what instruction will be required and what family assistance will be available in caring for the consumer, as well as what other Agency and community services will be requiredAssures that the physician's orders are appropriate and discusses necessary changesAssists in development and implementation of the interdisciplinary consumer care plan to include Occupational Therapy in consultation with the physicianGuides and instructs the consumer in prescribed therapeutic self-care and creative activities that are directed toward improving independence and physical and mental functioningInstructs consumer in care and use of wheelchairs, braces, splints, and prosthetic and orthotic devicesTeaches, supervises and counsels the family and consumer in the total Occupational Therapy program and other related problems of the consumer at homeEstablishes goals based on the consumer's current level of functioning and potential for improvement and performs re-evaluations as indicatedMakes arrangements for outpatient services when procedures are required that cannot be given in the homeEvaluates the effectiveness of occupational therapy intervention and updates the consumer care plan as neededPrepares clinical/progress notes on the day of the visit which are incorporated into the chart per agency policiesCommunicates with the physician on a regular basis regarding consumer's status, obtaining additional orders, if necessaryCommunicates with other team members and, when appropriate, instructs them in Occupational Therapy techniques that they may use while working with the consumerSupervises the CNA per agency policy when Occupational Therapy is the only professional discipline involved in the consumer's careSupervises the Occupational Therapy Assistant per agency policyParticipates in Agency activities, in-services, and meetings, when appropriateConfirms, on a weekly basis, the scheduling of visits with the Nursing Supervisorin order to coordinate Agency visitsParticipates with the staff, the consumer, and the physician in discharge activities and completes an Occupational Therapy Discharge SummaryAssumes responsibility for self-development by continually striving to improve his/her health care knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organizations and individual research and readingASSISTS IN THE AGENCY ADMISSION PROCESSObtains a medical history, particularly as it relates to the present conditionEvaluates the consumer, the consumer's significant other(s) and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the consumer, and what other Agency and community services will be required.Assures that the physician's orders are appropriate and discusses necessary changes.JOB CONDITIONSThe ability to drive and the ability to access consumers' homes which may not be routinely wheelchair accessible are required.Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the consumer's condition and to perform consumer care.On occasion, may be required to bend, stoop, reach and move consumer weight up to 250 pounds; lift and/or carry up to 30 pounds.The ability to communicate well, both verbally and in writing in English is requiredEQUIPMENT OPERATIONOccupational therapy equipment and other medical equipment will be utilized. Hand washing materialsCOMPANY INFORMATIONHas access to all consumer medical records which may be discussed with Nursing Supervisor, the Administrator, the Executive Director, and the Quality Assurance department staff and government agenciesRequirements:QUALIFICATIONSMust be a graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy AssociationPreferably should be a member of the American Occupational Therapy AssociationMust be licensed in ColoradoMust have at least one year of institutional/home care experienceMust have a criminal background checkMust have current CPR certification
