JOB SUMMARYUnder the direction and supervision of the Hospice R.N. Case Manager, provides skill nursing care, personal care, emotional support, and assistance with activities of daily living to terminally ill patients in the home, the majority of who are over the age of eighteen.This position is responsible for assisting in the implementation of the patient's care plan and assisting the patient in maintaining good personal hygiene and a safe and clean environment.
