Must be comfortable driving in snow and ice.*Licensed Practical NurseProvide direct nursing care under the supervision of an RN Clinical Manager of shift and/or Director of Nursing.Making frequent rounds to evaluate the resident status.Passing medication to patients as required by their physician.Providing wound care.Documenting care that is provided and assure that practices are consistent with facility policies and procedures.Oversee nursing assistants and ensure assignments are completed.Assume the position of team leader of unit and act as liaison between nurses, nursing assistants and clinical manager.Plan nursing care on unit assigned for 8 hours, up to 12 hours. (Maintain high nursing standards).Assists in performing nursing assessment: Performs focused screening/assessments/observational assessments.Initial, admission or event-focused assessments (i.e., incident reports)LPN's are to make our incident reports on residents, staff, visitors and medication errors as indicated. Give emergency tx if needed. Notify the MD and administrator if indicated. If indicated, make the necessary arrangements for transportation (i.e., ambulance transfer) and notify the DON or designee.Collects data and information and communicates outcomes of the data collection process in a timely manner.Contributes to the development of the nursing care plan and attends care plan meetings.Communicates with medical providers.Communicates with family members regarding condition changes, medications changes, behavioral concerns.Notifies MD of change in residents' conditionNotifies funeral director in case of resident death.Make out transfer forms and complete all arrangements for the transfer.Make up work schedule for nurse assistants on unit and explain the special needs of the residents. Answer any questions from nurse assistants. Consult with Clinical Manager, as necessary.Assist in giving ADL nursing care when needed.Set up, administer, and chart medications in ECS program. Reorder and check new meds. on electronic MARS/TARS. Order medications by faxing or calling the pharmacy as needed.Do treatments and chart according to electronic TAR.Initiate starting up of machinery such as nebulizers, oxygen, suction, etc.Do daily charting as assigned in ECS & as needed, such as fall follow-ups and change of conditions.Supervise the care of critical residents and help with care as time permits.Assist the Clinical Manager with evaluation of nursing assistants and new LPN's.Take part in nursing conferences, neighborhood teaching and team meetings/in-service programs.Shall accept responsibility of shift supervisor, when assigned, to care, keys, change schedules, as needed, and call Clinical Manager on 2nd or 3rd shift.
