This is a working manager role: you will carry a patient panel, provide day-to-day oversight of a team of Community Health Workers, and grow into leading a team of RNs as the organization scales.You will be a key clinical voice in how case management is delivered at Chamber: managing your team and patient panel with consistency, providing input on care workflows and protocols, and helping embed NCQA-compliant practices as we scale. This role requires someone who thrives in ambiguity; programs, workflows, and team structure will evolve, and you'll need to evolve with them.Key Responsibilities:Stand up RN case management workflows across core programs (e.g., transitions of care, longitudinal care management)Personally manage a panel of patients to set the clinical and operational standardIdentify early gaps in care delivery, staffing, and workflows and solve them in real timeProvide day-to-day oversight of a team of CHWs from day one, including performance management and escalations. Grow into leading FTE care staff as the team expands, including RNs and Care ManagersHire, onboard, and develop Care Managers and RNs as the team growsProvide input on role design and capacity needs as the team expandsAdapt quickly as team composition, workflows, and program priorities shift; This is a build environment, not a steady-state oneBuild training and onboarding programs that ensure consistency and qualityMonitor team and patient-level performance against defined metrics (e.g., engagement, LOS, readmissions, care progression)Partner with Program Operations on performance reporting and continuous improvement; surface clinical insights and team-level trends that inform decisionsUse data to drive continuous improvement across teamsServe as an escalation point for complex clinical and operational casesModel strong clinical judgment and reinforce best practices across the teamEmbed NCQA-compliant workflows and documentation into daily operationsPartner with Program Operations to provide clinical input on workflow design, documentation standards, and AI-enabled care processesPartner cross-functionally to integrate workflows with product, data, and payer requirementsWhat Success Looks LikeCHW team has a clear daily operating rhythm with defined expectations and a reliable escalation path from day oneActively managing a patient panel and operating as a consistent clinical presence the team can lean onConsistently models adaptability and brings the team along through change without losing clinical or operational momentumWorkflows and documentation practices reflect clinical input you've provided in partnership with Program OperationsAs the function grows, care staff under your management are performing consistently and patients are progressing through care pathwaysLong-term: a high-performing RN team with strong clinical outcomes and accountability structures you helped shapeRequirements:Compact RN license required; BSN preferredCertifications: ACM, CCM, or CMAC preferred58+ years of case management or care coordination nursing experiencePrior experience in a startup or high-growth environment; must be comfortable operating in ambiguity, adapting quickly as programs evolve, and building in motionExperience managing teams or contracted staff, including day-to-day oversight, performance management, and escalationsComfortable operating as both a clinician and a people manager simultaneously -- this is a working manager role from day oneExperience in value-based care, population health, or managed care preferredStrong operator with a bias toward action, ownership, and continuous improvementLocation:This is a remote role, but you must be based in the United States.
