The specialist prioritizes medication histories, gathers comprehensive medication information from multiple sources, and documents details thoroughly to support safe patient care.Key Responsibilities:Navigate electronic medical records to identify and complete medication histories for admitted or pending patients.Collect detailed medication information through patient interviews, healthcare providers, pharmacies, and other care facilities.Compile accurate medication lists including drug names, dosages, routes, frequencies, indications, and last doses taken.Document medication histories and allergy information promptly and accurately in the electronic system.Escalate unresolved medication issues to pharmacists and admitting providers.Maintain patient confidentiality and secure sensitive information at all times.Report safety hazards promptly to supervisors or appropriate personnel.Conduct educational sessions for technicians, nurses, and other healthcare staff, including patient education.Precept and train pharmacy technicians, students, pharmacists, and nursing staff on medication history processes.Assist with data collection for quality improvement initiatives.Education:Completion of Pharmacy Technician Certification Board (PTCB) certification required unless currently enrolled in a PharmD program.Registration with the state Board of Pharmacy is mandatory.Experience:Minimum of 6 months to 1 year experience as a pharmacy technician.Preferred: 2 years of retail pharmacy experience.Skills and Abilities:Strong attention to detail and technical proficiency.Knowledge of common medication doses and forms preferred.Excellent interpersonal and communication skills to interact effectively with patients and healthcare teams.Ability to prioritize tasks in a high-volume, dynamic environment.Adaptability to changing workloads and schedules.Physical Requirements:Ability to work independently and manage stress.Capability to read physician notes, medication labels, and lists accurately.Manual dexterity for operating pharmacy equipment and technology.Ability to stand and walk for extended periods (6-10 hours) and lift moderate loads (up to 10 lbs), including stooping and kneeling.Effective verbal and written communication skills are essential.This position serves a diverse regional population in the Southeastern United States, providing vital pharmacy support within a collaborative healthcare environment.
