Travel Physical Therapist - Skilled Nursing Facility

American Traveler is seeking a compassionate and skilled Physical Therapist (PT) for a 13-week assignment in a Skilled Nursing Facility (SNF) setting, with experience in geriatric rehab preferred.Job Details• Skilled Nursing Facility (SNF) setting, encompassing inpatient, long-term care, and short-term rehabilitation,• Primary patient population includes geriatric residents with mobility limitations, functional deficits, and varying levels of independence,• Day shift schedule, 9AM-5PM,Job Requirements• Graduate of an accredited Physical Therapy program,• Current, active state PT license required; license required if applicable,• CPR certification required or preferred depending on facility policy,• Experience in a SNF or geriatric rehab setting preferred,• Strong communication, documentation, and interpersonal skills,• Ability to work independently and as part of a multidisciplinary team,• Must be able to perform physical tasks including lifting, standing, walking, and assisting patients with transfers and mobility,Additional Information• Responsibilities include conducting thorough patient assessments, developing and implementing individualized treatment plans, and delivering evidence-based PT interventions targeting strength, coordination, balance, and mobility,• Collaborates closely with physicians, nursing staff, occupational and speech therapists, and family members to coordinate comprehensive patient care,• Required to monitor and document patient progress in compliance with state, federal, Medicare/Medicaid, and facility guidelines,• Participates in care planning meetings and interdisciplinary team conferences,• May supervise PTAs and therapy aides as needed,
