CASE MANAGER Day Programs
The function of our Case Managers is to plan, coordinate and implement program services to assist consumers to obtain optimal functioning.
Duties and Responsibilities:
- Assist Assistant Program Manager in the instruction and training of staff on site
- As part of the Program Admissions Team, evaluate prospective consumers for admission. Collaborate within the team to develop the initial service plan and initial program schedule for each new consumer.
- Collaborate with the consumer, staff, and significant others to develop each Individual Service Plan.
- Assist Assistant Program Manager and direct staff in developing resources and in scheduling daily activities relative to consumer goals and Individual Service Plans.
- Assist consumers with concerns relating to their daily activities. Act as a liaison to care providers, staff and regional center case workers.
- Working with other members of the Admissions Team, assist in the development, implementation and monitoring of informal behavior intervention plans, vocational training plans and community integration plans.
- Assist in review and recommendations of any serious incidents.
- Assist in assuring quality services as per program design.
- Attend staff meetings, consumer support meetings, and other meetings as assigned.
- Work with staff, residential care providers, and other agency case managers to assess consumer progress, evaluate effectiveness of service plans and adjust as appropriate.
- Ensure coordination of program services; prepare written assessment, program plan, and evaluation of consumer assigned caseload.
- Monitor consumer health, medical, and behavioral concerns.
- Maintain professional standards and conduct as case responsible person.
- Set up and maintain consumer records.
- Responsible for safety of all persons assigned.
- Adhere to all safety regulations.
- Transport consumers as needed.
- All other duties and responsibilities assigned.
Authority: As assigned by Program Manager, within scope of Program Manager position.
Supervised By: Program Manager, Assistant Program Manager
Requirements:
Preferred B.A. degree in an appropriate field or equivalent, with a minimum of one years’ experience in rehabilitation or related field and demonstrated knowledge of behavioral technology; oral and written communication skills required; current CPR and First Aid training required. Must have a clear driving record, valid CA driver’s license, 5 years driving experience, safe reliable transportation, and proof of current vehicle insurance. Able to lift up to 25 lbs. by self; up to 150 lbs. with one other person; able to push wheelchair with consumer up to one mile; standing walking, sitting, pushing bending, pulling required; squatting and repetitive motion may be necessary. Proper handling/use of chemicals for cleaning and work crews may be necessary, use according to manufacturers labeled instructions.
Technological Requirements:
Must possess and demonstrate a basic level of proficiency in Microsoft Office, including Word, Excel and Outlook.

Pay: From $21.88 per hour
