Program Instructor
Summary
At Creative Minds our mission is to assist individuals with developmental disabilities be as Independent as possible. We currently have a few positions open which include Activity Instructors for our Adult Day Program (ADPI), Adult Day Program Lead (ADPL), and Tailored Day Program Instructor (TDPI). All positions will provide services on our campus, in the community, and where alternative services can be provided.
ADPI & TDPI Essential Duties and Responsibilities are:
Under the direction of the Program Director and Supervisor, core duties and responsibilities include the following. Other duties may be assigned.
Support consumers in meeting the objectives outlined in their individual service plans.
Review consumers’ initial assessment and individual program plan.
Provide consumers with the supports necessary to meet their goals and objectives.
Monitor consumers’ safety in the facility and in the community.
Teach and develop lesson plans for consumers based on the consumers’ individual service plans.
Plan and prepare weekly classes and activity schedules including community integration opportunities.
Plan individual and group activities to stimulate growth in language, social, and motor skills such as learning to listen to instructions, interact and socialize with others, and using activity equipment.
Provide data collection related to individual consumers that reflect progress, problems, solutions and other pertinent data by maintaining and updating the monthly progress logs and daily case notes.
Attend initial orientation, all in-service trainings and staff meetings.
Keep working areas clean and organized.
Maintain all equipment and materials assigned.
Assist consumers with meals and refreshments during their lunch and as needed in the facility and during community outings.
Pay range: $18.00-$19.00

ADPI and TDPI position responsibilities:
Transport consumers to activities in the community (as designated).
Transport consumers from and to their residence before and after program hours (as designated).
Position requires the following:
Must have a Valid California driver's license
Must be meet company insurance requirements and have a clean driving record
Must pass pre-employment and maintain a clean criminal background screening / fingerprint clearance while employed
Must pass health screening and TB testing
First aid and CPR certification (training is provided by employer)
Fingerprinting
