Please note that no on-call or weekend work is required.We are seeking a provider for a 4 days per week (32 hours) position split between two centers located in Frederick, MD and Walkersville, MD: Autumn Lake Healthcare at Ballenger Creek and Autumn Lake Healthcare at Glade Valley. This position can be filled by a nurse practitioner or physician assistant. The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The Advanced Practice Provider collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities. The Advanced Practice Provider collaborates with the Medical Director and/or Attending Physician for patient‑specific medical care.The Advanced Practice Provider provides primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center. The Advanced Practice Provider works collaboratively with Center Leadership to achieve AlignMed Partners’ mission of improving the lives we touch through the delivery of high‑quality health care and everyday compassion.Qualifications
The Advanced Practice Provider must be a graduate of an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner by graduating from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing and a concentration in Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations/specialty tracks will be considered based on experience and business need. The Physician Assistant must be a graduate of a program accredited by the ARC‑PA and must pass the PA National Certifying Exam (PANCE) to use the PAC designation. PA state licensure required; NCCPA certification required.A minimum of two years of experience working with adult or geriatric populations is preferred. In states where practice is allowed, APRNs must have an unrestricted professional license and DEA for each state in which they practice. PAs must also have an unrestricted professional license and DEA for each state in which they practice.Benefits
