We are committed to strong clinical standards, consistent operations, and long-term partnerships that support meaningful patient outcomes.Physical Therapist (PT) - Mapleton, MNBlue Stone Therapy is seeking a Physical Therapist (PT) to provide rehabilitation services to adult and geriatric patients at Mapleton Community Home - Mapleton, MN.This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapist is responsible for patient assessments, care plan development, and implementation of treatment programs designed to improve gait, balance, mobility, strength, range of motion, pain management, and overall functional independence.This position may serve multiple partner communities, including St. Luke's Lutheran Care Center - Blue Earth, MN and Whispering Creek - Janesville, MN, with travel between locations as needed.$10,000 sign-on bonus available for eligible full-time candidates.What You'll DoPerform physical therapy evaluations and develop individualized plans of careProvide skilled physical therapy treatment focused on functional and measurable outcomesAddress gait training, balance, mobility, strength, range of motion, pain, and discharge planningSupervise Physical Therapy Assistants in accordance with licensure and regulatory standardsMaintain appropriate patient schedules per established guidelinesComplete timely, accurate documentation to support quality care and reimbursement requirementsCollaborate with interdisciplinary team members and facility staff to support positive patient outcomesRequirements:What You BringGraduate of an accredited Physical Therapy programCurrent Physical Therapist license (or eligibility for licensure) in applicable stateExperience in skilled nursing, long-term care, or post-acute rehabilitation preferredStrong clinical reasoning, communication, and organizational skillsCommitment to patient-centered care and professional standardsWhy Blue Stone Therapy?We believe exceptional care begins with exceptional clinicians.
