CNA - Certified Nursing Assistant

TITLE: CNA POSITION SUMMARY: This is a position with responsibility for the health needs of the residents and support of the assisted living care team. This position is responsible for the health needs of the resident and maintaining the well-being of the residents. It includes planning, implementing, recording and problem solving both the medical and social needs of the residents in order to maintain residents at their highest functional level. ESSENTIAL RESPONSIBILITIES: Responsible for assessing and monitoring the health living residents to su port tie resident's overall quality of life. If delegated, will assist DON in resident assessment and the development of individualized service plans (ISP). Provides technical support enable the care teams in the execution of resident's ISP. Maintain resident health status records in compliance internal policies. Will monitor the well-being of dementia residents by reviewing resident daily notes, noting in resident conditions, will follow-up on resident changes. Recognize and interpret change-in resident condition, notify Executive Director and physician of condition change and take appropriate actions as directed by physician, manager, and/or families/guardian. Monitor resident weights and blood pressure and, if necessary, resident nutritional intake. Monitor infection control procedure to ensure compliance to infection control policy. Maintains on-going communication with Executive Director about the well- being of the residents. Coordinates the resident's internal and external healthcare services. Develops and maintains excellent family relationships. Participates in staff and in-service programs. Responsible for maintaining resident dignity and respect. Other duties as requested. QUALIFICATIONS: Experience working within a social model care program is required and a understanding of person centered care is essential. Relevant experience working in public home health, or assisted living is preferred. PHYSICAL REQUIREMENTS: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Must be ambulatory to move around the community, grounds and to carry out assignments. Able to lift up to 50 lbs., bend to the floor, reach above head level, sit, to perform repetitive movements with arms and legs and to be on one's feet for extended periods of time. While performing the duties of the job, the employee may perform extensive keystroke tasks. WORK ENVIRONMENT: The requirements and work environment described below is representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee could be exposed to healthcare environment with potential exposure to blood and body fluids.
