WORK FOR US!PrimaryOne Health is currently seeking a Patient Navigation Licensed Practical Nurse for our call center. Essential Job Responsibilities: Performs all functions and job responsibilities of a Patient Navigation Specialist.Performs short evaluation of the patient situation to estimate severity of illness and/or injury including learning about chief complaint and, as possible, obtaining vital sign and mental status information.Talks directly to patients on the telephone and then directs them to emergency rooms (EDs) or urgent care centers or to see their physician during office hours.Case manages patient's no-shows, chronic disease managements and help in system navigation as well as care coordination.Advise and provide patients with other services offered by PrimaryOne Health.Determines urgency of seeing the patient based on brief assessment and on familiarity with a patient's condition and history, per protocol. May use computerized medical information database, which uses algorithms that closely imitate physician logic and thought patterns, as guide. Confers with physician as needed.Sends those with high-risk chief complaints such as chest pain, abdominal pain, or severe headaches to ER immediately or arranges for ambulance per protocol. May provide appropriate home health advice to those patients who do not need to go directly to the ER.Sets up appointments for patients who do not need to go to ER but need to see a physician.Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients.Other duties or special projects as assigned. Successful candidates must possess a current LPN state license and BLS; on-the-job training in triage. Minimum two years of experience as a nurse; one to two years of telephone triage experience, preferably in medical practice setting, and call center experience.
