If you are dependable, mission-driven, and passionate about patient care and support, we invite you to join our team.Compensation$15.75-$22.50$1/hr shift differential for nightsBenefits PTO (Full-Time and Part-Time employees only) Medical, Dental, and Vision (Full-Time only) Employee Assistance Program (EAP) 401(k) OptionShifts: PRN Nights, must provide at least 12 hours of patient care every 30 daysKey Responsibilities Assist with activities of daily living (ADLs) (bathing, dressing, grooming, toileting,mobility) Provide companionship, emotional support, and social engagement Support meal preparation, nutrition support and light housekeeping Monitor and report changes in condition Follow care plans and document accurately using EMR standardsQualifications High school diploma or GED Active CNA certification Colorado CPR certified Valid driver's license and reliable transportation Ability to work independently in a non-facility setting Home health, long-term care, or hospice experience preferred.
