Job DetailsDescriptionPosition SummaryChicago Commons is seeking a Program Coordinator - Senior Services (Caregiver Support & Outreach) to support the Senior Services Division, including the City of Chicago Caregiver Support Services grant. The Caregiver Support Services grant helps family caregivers access city services and navigate the challenges of caring for loved ones with dementia related or disability diagnosis. This role is pivotal in providing essential services and resources to caregivers in our community. The coordinator will be responsible for various aspects of the programs, including but not limited to outreach, referral management, facilitating workshops/support groups, and providing support for TCARE assessments. This position will report to the Program Director – Senior Services to assist in ensuring quality services are provided and the program is successful in achieving intended goals.Program GoalsIncrease awareness and empower caregivers.Identify resources and social services that support the caregiver and their loved ones.Delay premature institutionalization of their loved ones.Alleviate caregiver stress and social isolation.Education & Certification Requirements2 or more years’ experience with program coordination, administration, social services, counseling, or related.High school diplomaValid driver’s license and reliable transportation for conducting in-home assessments.Previous experience with facilitation of support groups Experience & Training RequirementsExcellent written and verbal communication skillsMust be proficient in Microsoft Office Suite.Strong data entry and database management skills.Ability to work independently as well as in a team environment.Ability to communicate effectively and appropriately with others.Must be able to maintain confidentiality.Must be able to travel to Commons sites and clients’ homes when needed.Required to speak and read English proficiently.Bilingual (Spanish & English) preferred.Desired OutcomesAbility to provide timely outreach to clients and maintain appropriate records.Perform in-home assessments with care and accuracy.Facilitation of support groups and workshop events.Essential Job Requirements & DutiesCoordinate outreach efforts to identify and engage caregivers in the community, ensuring inclusivity and accessibility of services.Provide caregivers with information and resources related to caregiving, including educational materials, support services, and community resources.Collaborate with community organizations, healthcare providers, and government agencies to enhance support services for caregivers.Schedule and conduct in-home assessments to evaluate the needs of caregivers and their care recipients.Maintain accurate and up-to-date records of all clients, including program participation, assessments, and services provided.Support the evaluation of program effectiveness through feedback, survey administration, and making recommendations for program improvements.Ensure confidentiality and compliance with data protection regulations.Assist in the planning, organization, and execution of workshops, support groups, and other events aimed at supporting caregivers.Coordinate logistics, including venue selection, materials preparation, event setup, and participant communication.Perform general administrative duties, including scheduling appointments, answering inquiries, and maintaining event / workshop supplies.Maintain regulatory compliance with city, state and federal guidelines.Identify gaps in services and make recommendations to the Program Director.Support the Program Director and Chief of People and Culture with other tasks as needed.Physical RequirementsThe employee frequently is required to stand, walk, reach with hands and arms, and stoop, kneel, crouch, or crawl. Prolonged periods of sitting at a desk and using a computer. Ability to hear and speak clearly for telephone and in-person conversations.
