Smiles Like Yours, LLC is hiring for a Residential Coordinator/In-Home Support Coordinator for its group homes in the Chester/Midlothian/Henrico areas. The In-Home Support Coordinator is responsible for overseeing the fulfillment of Company goals related to programming and compliance and serving as House Manager for homes under their purview. The In-Home Support Coordinator coordinates activities for the Company's residential program and serves as a liaison between the Company and service-involved individuals. This position supervises employees carrying out the daily functions for Divisions within the Company that provide care inside a home or the home of the individual(s).Duties include, but are not limited to:· Implementing and following the policies and procedures of the Company.· Applying first aid, CPR and or Tova, when necessary.· Developing, reviewing, and signing Individuals Support Plans.· Conducting yearly admissions and discharge meetings as needed.· Processing referrals and billable hours, when applicable.· Monitoring progress quarterly.· Maintaining and securing all files and records.· Supervising Direct Support Staff; coordinates work schedules.· Preparing and submitting reports on behalf of the Company.· Responding to inquiries from Case Managers, family members, and other involved individuals.· Administering medication, when necessary.· Performs other related work, as required.Minimum knowledge of: Characteristics and concepts of developmental disabilities, mental health, health disorders and related physical conditions and treatment approaches.Knowledge of simple medical care, first-aid, behavior management.Knowledge of developing Individual support plans.Conducting initial and comprehensive assessmentsMinimum Skills and abilities:· Ability to enter service authorizations in WAMS.· Ability to operate a computer system.· Ability to communicate professionally to staff, individuals, support coordinators and authorized agents.· Ability to ensuring the fundamental rights of each adult are protected.· Develop initial assessments and Comprehensive assessments.· Develop Support Activities for Individuals support Supervising and monitoring individuals and their engagement in meaningful activities that address ISP outcomes.· Residential Coordinator demonstrates a working knowledge of the policies and procedures that are applicable to the position.
