Outreach Worker Holyoke Job Category: Direct Service Requisition Number:OUTRE005912 Full-Time Rate: \$20 USD per hour Adult Mental Health -Springfield Springfield, MA 01109, USA Job Details The Center for HumanDevelopment, (CHD) is seeking an Outreach Worker for our Adult CommunityClinical Services (ACCS), located in Springfield, MA. ACCS is fundedthrough the Department of Mental Health and is a multidisciplinaryoutreach approach for persons served in need of psychiatric and otherservices living independently in the community. This involves activeengagement including assertive outreach, consistent assessment andtreatment planning, skill building, symptom management, addictiontreatment, and behavioral and physical health monitoring and support.YOUR ROLE AS AN OUTREACH WORKER: ACCS teams include licensed clinicians,RNs, housing specialists, substance use specialists, direct care staff,and peer/family specialists. The Team works collaboratively to provideperson-centered, evidence-based services in support of Persons\'treatment goals. The Direct Care Staff assists participants in need ofpsychiatric services acquire the skills necessary to live independentlyin the community. S/he works within the Agency\'s team model,establishing open communication, support, and accountability amongco-workers. REQUIRMENTS: At least one year experience working withpopulation preferred Bachelor\'s degree from an accredited educationalInstitution Preferred (required For DC II, Or Additional Experience)High School diploma required ESSENTIAL JOB FUNCTIONS: Assistsparticipants with activities of daily living e.g. personal hygiene, mealpreparation, housekeeping, laundry, money management, etc. Transportsparticipants in matters related to service delivery, e.g. medicalappointments, shopping, job interviews, vocational program, banking,laundry Administer medication accordance with MAP policies andprocedures Maintains records in accordance with standard format andtimetables to include assessment and progress reports/service notes,incident reports, money disbursements, and other specializeddocumentation as required Provides for and participates in recreationalactivities, e.g. dining out, spectator sports, movies, etc. for theparticipants Participates in the development of treatment plans of thoseserved Pay Rate: \$20/hr This schedule is Tuesday-Saturday - Full time40hrs. /week.
