You will have the opportunity to make a positive impact on the lives of patients in the comfort of their own homes while collaborating with an experienced and supportive healthcare team.As an Occupational Therapist Team 2 you will work in multiple locations including: Atlantic Beach, Baldwin, Cedarhurst, East Rockaway, Bellmore, Freeport, Hewlitt, Inwood, Island Park, Lawrence, Levittown, Lido Beach, Long Beach, Lynbrook, Merrick, North Bellmore, North Merrick, Oceanside, Point Lookout, Rockville Center , Valley Stream, Wantagh, Woodmere.Job DetailsKey Responsibilities:* Conduct assessments of patients' physical, cognitive, and emotional needs in their home environment.* Develop and implement personalized treatment plans to improve patients' ability to perform daily activities.* Work collaboratively with other healthcare providers, including physicians, nurses, and home health aides, to deliver coordinated care.* Educate patients and families on therapeutic techniques, adaptive equipment, and ways to enhance the home environment for safety and independence.* Document patient progress, treatments, and goals according to regulatory requirements.* Participate in team meetings to discuss patient progress and treatment plans.Salary RangeUSD $90,000.00 - USD $115,000.00 /Yr.Qualifications:* Education: Bachelor's or Master's degree in Occupational Therapy from an accredited program.* Licensure: Active and valid Occupational Therapist license in New York State.* Experience: Prior experience in home care is preferred but not required. New grads are welcome to apply.* Skills: Strong interpersonal, communication, and problem-solving skills. Ability to work independently while managing patient caseload.* Other: Reliable transportation to travel to multiple locations in the designated area.
