Job Description SummaryThe Mobile Nurse Practitioner provides comprehensive primary and/or specialty healthcare services to patients in facility-based settings including Board & Care homes (B&C), Assisted Living Facilities (ALF), and Skilled Nursing Facilities (SNF). This is a mobile role responsible for delivering patient-centered care directly in the field across Los Angeles and Orange County, with an immediate focus on the San Fernando Valley and West Los Angeles areas. The Nurse Practitioner functions as a primary care provider and collaborates with an interdisciplinary healthcare team to assess, diagnose, treat, and manage patient health needs.How will you make an impact & RequirementsThe Mobile Nurse Practitioner delivers high-quality clinical care to patients residing in long-term care and residential healthcare settings. This role requires strong clinical judgment, autonomy, and the ability to effectively coordinate care with other healthcare professionals.Key responsibilities include:Serve as the primary care provider for patients within assigned facilities, including B&C homes, ALFs, and SNFs.Conduct comprehensive patient assessments, evaluations, and follow-up visits in a mobile/field-based environment.Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical conditions and healthcare needs.Diagnose and manage acute and chronic conditions while developing appropriate treatment plans.Prescribe medications and other therapeutic treatments in accordance with state regulations and clinical guidelines.Develop, implement, and document individualized patient care plans.Provide health education, disease prevention counseling, and lifestyle guidance to patients and caregivers.Collaborate with physicians, nurses, and other healthcare professionals as part of an interdisciplinary care team to coordinate patient care.Maintain accurate and timely clinical documentation in accordance with regulatory and organizational standards.Travel to assigned facilities within Los Angeles and Orange County, with current coverage needs in the San Fernando Valley and West Los Angeles regions.Minimum Requirements:Master of Science in Nursing (MSN) from an accredited program.Current certification as a Nurse Practitioner.Minimum of one (1) year of clinical experience as a Nurse Practitioner or related advanced practice clinical experience, or an equivalent combination of education and experience.Active, valid, unrestricted Registered Nurse (RN) license and Nurse Practitioner (NP) license in the state of California.Active California furnishing number and DEA registration required.Ability to travel within assigned service areas in Los Angeles and Orange County.Satisfactory completion of a Tuberculosis (TB) test is required for employment.Compensation:$119,313.00to
