Lifekind Health is looking for a Care Coordinator to join our team in San Jacinto. Ensure that patients receive comprehensive and continuous healthcare services tailored to their individual needs. Act as a liaison to facilitate communication among multidisciplinary teams, ensuring that care delivery is seamless and efficient.
