If you are looking for a role that helps support meaningful, patient-centered care, this is your opportunity to make a difference!How You’ll Make a Difference (Essential Functions)The position, HIM Specialist, is responsible for providing customer service regarding patient chart requests. The HIM Specialist is also responsible for providing technical and administrative support related to obtain, maintaining, and releasing patient health records.Key ResponsibilitiesCustomer ServiceCommunicate via phone, fax, and email with internal and external customers. Assist staff with HIM questions, issues or requests related to the patient chart, e.g. request for records from external agencies. Technical & Administrative SupportProcess documentation, including staff notification of receipt. Prepare, scan and index documents into the electronic health record (EHR.). Maintain in-depth working knowledge of HIPAA requirements and related Agrace policies and procedures, to facilitate the release of necessary information for treatment, payment and operations requests from partnering facilities. E-fax physician orders for signature. Communicate and follow-up. Enter data into EHR, scan or file returned documentation. Monitor EHR for missing forms and other objective deficiencies. Follow up with staff and clinical leadership to resolve identified issues. Assist with patient chart audits at admission, discharge and/or death. Other DutiesOversee the work of HIM volunteers. Participate in the design and implementation of health information management practices and protocols. Participate in internal work groups, meetings and committees relative to position responsibilities. The above are not to be construed as exclusive or all-inclusive.
