Hospice RN Case Manager
Hospice Services of Massachusetts is seeking a licensed and experienced RN Hospice Case Manager to join our team in the North Easton area. As an RN Hospice Case Manager, you will be responsible for supervising hospice aides, conducting comprehensive assessments of patients and families, and providing professional nursing care to patients in their homes and communities. You will also participate in revising the plan of care as necessary and use health assessment data to determine nursing diagnosis.$42 to $52 an hour with weekly pay.
