Job Description Summary: The Quality Outcomes Specialist, under the direction of the VP Hospice Clinical Operations, assesses and facilitates clinical systems and/or processes to support the delivery of high-quality care and achievement of quality outcomes that exceed national benchmarks. Identifies outcomes variances and takes initiative for timely resolution of negative trends. The Quality Outcomes Specialist can synthesize an analysis of complex systems, develop and implement solutions to improve processes and meet goals. Supports the organization-wide clinical operations goal and objectives and quality assessment/performance improvement program.Job QualificationsEducation: Graduate of an accredited nursing program and possession of a current, active LPN or RN license in good standing with the applicable state licensing board.ExperienceKnowledge of Hospice federal and State regulatory standards as they apply to performance improvement and applicable accreditation standards.Ability to develop and maintain interpersonal relationships with regional and branch leadership.Ability to effectively communicate both verbally and in written forms.Ability to define problems, collect data, establish facts, and draw valid conclusions and evidence performance improvement via measurable results.Ability to enter and abstract data using personal computer, as well as the ability to utilize data to facilitate the improvement and change in processes.Ability to facilitate performance improvement teams, present data and promote a collaborative approach toward goal achievement. Ability to work independently and interdependently.Knowledge and application of process improvement tools and techniques.Prior Homecare Homebase experience preferred.Minimum three (3) years of Hospice/Home Health related experience.Essential FunctionsPerformance Improvement & Quality Champion for hospice publicly reported data/outcomes and internal key performance indicators (KIP’s).Supports the VP of Quality and Standards in the development and ongoing administration of quality assessments and chart reviews. Promotes organization wide understanding and adoption of the overall quality and principals of continuous performance improvement. Assists in the development of performance improvement/patient safety initiatives that support defined goals and objectives including data collection, analysis, and reporting of results. Conducts education as requested by staff or leadership. Coordinates teams as needed to achieve desired outcomes using appropriate performance improvement methodologies.Liaison to branch staff to support achievement of quality goals and monitoring activities. Facilitates a multi-disciplinary approach to improve clinical and quality outcomes. Monitors and evaluates clinical performance efforts using defined metrics (i.e. CAHPs for Hospice, Hospice Compare, Hospice Item Set). Makes recommendations to key leadership to achieve desired outcomes.Data Management, Analysis and Presentation - Abstracts pertinent information and enters data into databases using standardized methods and processes. Submits data within the required timeframe and follows through to ensure submitted data is accepted and processed accordingly and has no discrepancies. Collaborates with branch staff and leadership to timely correct any discrepancies or invalid submission. Routinely performs discrepancy management activities to maintain data integrity. Presents meaningful reports, analysis, and recommendations with measurement description, statistical information, and benchmarking information. Support improvement efforts for potential or actual quality of care issues including participation/facilitation of Root Cause Analysis (RCA) or event review as needed.
