As an essential member of a senior living and rehabilitation community, you will deliver high-quality, patient-centered nursing care to both adult and elderly residents within a nurturing environment.Key ResponsibilitiesProvide direct bedside nursing care to residents.Conduct thorough patient assessments to evaluate health status.Administer medications and treatments as prescribed.Monitor residents' clinical conditions closely to promote effective rehabilitation and long-term health.Collaborate with an interdisciplinary team, including physicians, therapists, certified nursing assistants (CNAs), social services, and family members, to create, implement, and regularly update personalized care plans that prioritize safety, comfort, and functional improvement.Ensure accurate and timely documentation in electronic medical records (EMR).Adhere to infection control and fall-prevention protocols.Manage pain effectively and coordinate transitions of care, including discharge planning and education.Communicate clearly and compassionately with residents and their families regarding care plans, medications, and ongoing health requirements.Requirements and QualificationsCurrent RN licensure.Strong clinical assessment and critical thinking skills.Ability to work collaboratively within a team-oriented environment.Excellent communication and interpersonal skills.Working ConditionsThis role offers the chance to practice professional nursing in a supportive, team-based setting that emphasizes clinical excellence and continuous learning.
