Licensed Practical Nurse

Job Summary: License Practical Nurse (LPN) perform a wide variety of job duties during their shifts, caring for and educating patients about their conditions, as well as establishing trust and building rapport with patients. The LPN is responsible for being part of the Medical team as well as helping to facilitate the flow of the Medical Department and Medication Administration Program.Duties/Responsibilities:Assumes responsibility and accountability for designated patientsProvides Nursing Care to all patients of all agesIdentifies self as nurse responsible for patient care to patients, family and physiciansProvides nursing care for all patients within scope of LVN practiceGathers data relevant to the patient's individual needsElicits important information concerning the patient's progressGathers data regarding patient needs while providing direct patient careDocuments nursing observations according to hospital charting standardsAdministers medications and treatments per company policyReports symptoms, reactions and progress of patient to the Medical Director and/or Clinical Director, and the treatment teamPursues proper chain of command to attain a satisfactory solutionInitiates changes to improve patient care; discusses changes with doctor when appropriateUtilizes educational opportunities within the hospital and other avenues to maintain clinical expertise to promote personal growth and developmentResponsible for knowledge of legal aspects of nursingRequired Skills/Abilities:Ability to work as a team member and have communication, organizational and interpersonal skillsAbility to work under stressful conditions and be flexible in relation to department needsKnowledge of DCF and TJC standards within the DepartmentAdhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to policies and agreements regarding confidentiality, privacy, and security.Demonstrates Proficiency in Verbal and written Communication SkillsKnowledge of State and Federal Statutes Regarding Patient Confidentiality LawsAbility to understand and abide by the Drug Free Workplace PoliciesAbility to complete the Harassment and Discrimination trainingKnowledge of Corporate Integrity and Compliance ProgramKnowledge of current technologyKnowledge of Substance Use Disorder. Can score above a 70% on the SUD TrainingFamiliar with community resources, or who in the facility to contact for community resourcesEducation and Experience:Valid LPN license, in good standing, in this State (unless in State with Nursing Licensure Compact)Experience in the field of chemical dependency/substance abuse/psychiatric setting or human services field is preferred.Complete TB testing upon being hiredComplete and be within the requirements of Background and Drug screening resultsComplete training or certifications in CPR, HIPAA, HIV/AIDS, Verbal De-Escalation, Harassment/Discrimination, Incident Reporting, Infection/STD Risks, HEP B/C, and Abuse, Neglect, Trauma, and Exploitation, Understanding SUD, and any other trainings required by the State or the Joint Commission.Physical Requirements:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
