Registered Nurse - Home Health (San Francisco Area - Geographies throughout "The Peninsula")Join a team committed to clinical excellence, professional growth, and helping patients achieve their rehabilitation and performance goals.Position Summary:The Registered Nurse provides skilled nursing care to patients in their home settings, assessing health conditions, developing individualized care plans, and coordinating comprehensive patient care. This role involves clinical assessment, direct nursing interventions, patient and family education, and collaboration with physicians and interdisciplinary healthcare teams to promote optimal health outcomes and independence.Key Responsibilities:Clinical Assessment & Care PlanningConduct comprehensive nursing assessments including physical, psychosocial, and environmental factors affecting patient healthEvaluate vital signs, symptoms, medication effects, and overall health statusDevelop patient-centered care plans in collaboration with patients, families, physicians, and healthcare team membersEstablish measurable goals and modify care plans based on patient response and changing needsDirect Patient Care & InterventionProvide skilled nursing interventions including wound care, medication management, IV therapy, catheter care, and ostomy managementMonitor patient conditions, identify changes in health status, and communicate significant findings to physicians and care teamAdminister treatments and medications as ordered, ensuring safe and appropriate deliveryImplement infection control practices and promote patient safety in the home environment Education & CollaborationEducate patients and caregivers on disease management, medication regimens, symptom monitoring, and health promotion strategiesCoordinate care with rehabilitation therapists, social workers, and home health aides to ensure integrated service deliverySupervise and provide clinical guidance to support staff in accordance with scope of practice and regulatory guidelinesParticipate in interdisciplinary case conferences and team meetingsCommunicate regularly with physicians and referral sources regarding patient progress and care needs Documentation & ComplianceMaintain accurate, timely documentation of all assessments, interventions, and patient outcomes in electronic medical recordsEnsure compliance with agency policies, professional nursing standards, state regulations, and federal healthcare guidelinesFollow payer-specific documentation and billing requirementsParticipate in quality assurance and performance improvement activities Professional DevelopmentMaintain active professional licensure and required certificationsComplete mandatory continuing education and competency requirementsStay current with evidence-based nursing practice and clinical advancements Qualifications: Education & LicensureAssociate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program; BSN preferredCurrent, unrestricted Registered Nurse license in CaliforniaCurrent CPR/BLS certificationExperienceMinimum one year of clinical experience as a licensed Registered NurseHome health or acute care experience strongly preferred Skills & CompetenciesStrong clinical assessment, critical thinking, and problem-solving abilitiesExcellent communication and patient education skills with diverse patient populationsAbility to establish therapeutic relationships and work compassionately with patients and familiesSelf-directed work style with strong organizational and time management capabilitiesProficiency with electronic medical record systems and digital documentationSound clinical judgment in managing complex cases independently Additional RequirementsValid California driver's license and reliable personal vehicleAbility to meet physical demands including patient transfers, equipment handling, and mobility assistanceSuccessful completion of background check and drug screeningPosition Type:
