Renal Dietitian FT

Ditmas Park Nursing & Rehab - Ditmas Park Dialysis Center is seeking a Registered Dietitian to support our patients' health and well-being.In this role, you'll work closely with our care team and patients to provide individualized nutrition plans that make a real difference in treatment outcomes and quality of life.Position OverviewAs a member of our care team, you'll play a key role in helping patients maintain optimal nutrition throughout their dialysis treatment. The ideal candidate will bring both clinical expertise and a patient-first approach, empowering individuals to achieve better health and improved quality of life.Key ResponsibilitiesConduct comprehensive nutritional assessments for dialysis patients.Develop and implement individualized meal and supplement plans based on labs, clinical needs, and patient preferences.Monitor and track patient progress, adjusting plans as needed.Educate patients and families on kidney-friendly diets, weight management, and feeding options including enteral nutrition.Collaborate with nephrologists, nurses, social workers, and other staff to deliver a comprehensive care approach.Participate in interdisciplinary rounds and contribute to patient care planning.Stay up to date with renal nutrition best practices and evolving standards of care.Provide mentorship and guidance to staff on nutrition-related practices.QualificationsCurrent Registered Dietitian Nutritionist (RDN) credential.Previous experience in dialysis or clinical nutrition preferred.Strong knowledge of renal, geriatric, and chronic disease-related nutrition.Excellent communication and interpersonal skills for patient education and motivation.Ability to perform nutrition-focused physical assessments and tailor interventions accordingly.Why Join Us?At Ditmas Park Dialysis Center, you'll be part of a collaborative and supportive team dedicated to delivering compassionate, high-quality care.
