Center with Labs, EKG and Echo'sPhysicians each supported by own scheduler, MA, referral specialistPatient Care team: Nurse, Case Manager, Social Worker & Outreach Specialist as well as a social community health coordinatorEach clinic/center invites patients for cooking classes, yoga, silver sneakers-mall walking exercise community for seniorsPACKAGESalary Range $250k for a resident.
