Social Care Network Navigator

Pay Range: $20-$23FLSA Status: Non-ExemptLocation: 899 Main Street, Buffalo NY 14203Summary Of DutiesThe SCN (Social Care Network) Navigator at BestSelf Behavioral Health plays a critical role in addressing social determinants of health by connecting individuals and families to essential community-based resources. The Navigator works collaboratively with clinical teams, community partners, and care coordination staff to reduce barriers to care and improve overall health and wellness outcomes.This position requires cultural humility, strong community knowledge, relationship-building skills, and a deep commitment to equity, dignity, and person-centered, trauma-informed care.Position ResponsibilitiesCoordinate outreach efforts, representing the agency at community events and partner sites managing logistics and outreach inventoryConduct screenings to identify unmet social needs (housing, food insecurity, transportation, utilities, employment, childcare, etc.).Develop individualized service plans in collaboration with clients.Connect individuals to appropriate community-based organizations and SCN partner agencies.Support warm handoffs and ensure follow-up to confirm service linkage.Document services, referrals, and outcomes in electronic systems in accordance with agency and SCN requirements.Maintain updated knowledge of community resources and eligibility criteria.Advocate on behalf of clients to reduce systemic barriers.Collaborate with teams internally and externally as appropriateMaintain compliance with HIPAA and confidentiality standards.Track data and assist with reporting requirements tied to SCN performance metrics.Ability to work independently as well as collaboratively with others.Completes all trainings required by the agency.Performs all other duties as assigned.QualificationsHigh School Degree or Equivalency is required.Minimum 2 years of experience in care coordination, case management, outreach, or community-based services.Knowledge of Social Determinants of Health (SDOH) and the impact of structural inequities on health outcomes.Experience with tabling, outreach, and interacting with various communitiesKnowledge of confidentiality standards and HIPAA compliance.Proficiency in Microsoft Office Suite (Word, Excel, Outlook, Teams) or equivalent platforms.Comfort using shared databases and community resource platformsAbility to meet specific deliverables as set in implementation planStrong communication, documentation, and organizational skills.Knowledge of local community resources and social service systems.Ability to lift and transport event materials (up to 30 lbs).Must have a flexible schedule with availability on weekends and evenings.Valid NYS Driver's License.Must have reliable access to a personal vehicle to conduct community visits and attend off-site meetings.Willingness and ability to travel throughout various service area(s) such as Erie, Chautauqua, and Cattaraugus counties as needed.Must have accredited NYS Defensive Driving course; or obtain within 15 days of hire and send certification of completion to Human Resources.Must maintain active NYS Defensive Driving course every 3 years and send a copy of the new certification of completion upon expirationMust maintain valid vehicle inspection, registration, and insurance cards and send updated copies to Human Resources upon expirationSome Things You Can Look Forward ToWelcoming, team environment, that inspires you to thrive and be your BestSelf!Rewarding work experience!Generous paid time offMany other unique lifestyle & personal insurance optionsCASAC certification tuition supportCareer growth and advancement opportunitiesWe look forward to telling you more!
