Home Health and Hospice RN - $66.42/hr

Job Summary (List Format):- Provide skilled nursing care and case management to patients in their homes or at the in-patient Hospice Center.- Admit patients onto hospice/home health services.- Coordinate and integrate services to ensure continuity of care.- Facilitate achievement of quality clinical and cost-effective outcomes.- Negotiate, procure, and coordinate necessary services and resources for patients and families.- Use the nursing process (assessment, planning, implementation, evaluation) for patient care, including family education.- Work collaboratively with an interdisciplinary team.- Observe, assess, and plan interventions for patients' physical and psychosocial needs.- Oversee and evaluate care provided by Home Health/Hospice Aides and CNAs.- Ensure continuity of care through effective communication and case conferences with other care providers and agencies.- Provide quality care and symptom management for optimal patient health and quality of life.- Maintain accurate and complete patient records per agency standards and OASIS requirements.- Participate in team scheduling, including on-call rotations and after-hours patient assessments as needed.- Engage in quality improvement activities and attend required trainings.- Perform other duties as assigned by supervisor.Minimum Qualifications:- Current RN license in Montana (BSN preferred).- Valid driver's license, auto insurance, and reliable transportation.- 2 years of recent acute care, home care, or hospice experience preferred (including venipuncture, catheterization, wound care, or parenteral therapy).Schedule:- In-home RN: Mon-Fri 8am-5pm.- In-patient RN: Three rotating 12-hour shifts.
