CarePlus TLC is seeking a dedicated, compassionate, CNA/Home Health Aid with a passion for serving others to join our team! Responsibilities:Take and record Vital signs including fluid intake and outputMonitor patient's condition and report changes/concerns to appropriate medical professionals and family membersProvide Activities of Daily Living care as outlined in the Care Plan-bathing, dressing, feeding, etc.Transfers patient with equipment using appropriate safety measures (gait belt, hoyer, sit-to-stand lifts)Perform light housekeeping duties to maintain a clean-living space for clients. Ability to turn and reposition patientsUnderstands First Aid and Emergency Care protocolsComplete electronic documentation while on shiftMedication remindersCatheter careSkills:Familiarity with Home Health Care, safety and sanitation standards and proceduresUnderstanding of Confidentiality and Privacy LawsExcellent communication and interpersonal skills… able to communicate effectively with patients and their families.
