Orthopedic Nurse Practitioner Per Diem Monday and Friday

Ortho Advanced Practice Nurse (APN) The Ortho Advanced Practice Nurse (APN) utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Preference for an experienced APP caring for Orthopedic patient population. The APN will perform orthopedic consults in ED and on the units as well as managing care for joint and spine program patients (Inpatient/Outpatient) Coverage needed is Mondays and Fridays. Responsibilities include performing and documenting accurate, complete, and relevant history and physical assessments appropriate for the patient's age, gender, and clinical problem; ordering, performing, and interpreting laboratory tests and diagnostic studies/procedures in collaboration with physicians and consultants as appropriate; managing general medical and surgical conditions based on comprehensive knowledge of etiologies, risk factors, pathophysiology, and the indications and contraindications of pharmacologic agents and other treatment modalities; prescribing medication safely and effectively in accordance with best practices, hospital policy, and NJ regulations, providing patient education as needed; seeking opportunities to promote Hackensack Meridian Health (HMH) APN practice recognition (locally, nationally, and internationally) through conferences, publications, and presentations; providing care that is patient- and family-centered, compassionate, and effective for the promotion of health and treatment of disease; practicing as a healthcare provider according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges; adhering to HMH nursing protocols, standards of care, and disease-specific quality indicators; applying evidence-based and value-based medicine to systematically analyze and improve patient care practices and outcomes; evaluating patient/family transition of care planning, considering resource utilization, cost, quality, safety and self-management promotion for chronic conditions; demonstrating clinical expertise through advanced assessment and procedural skills pertinent to the practice specialty; completing medication reconciliations as appropriate; participating in annual APN peer review; translating `best practice' into clinical practice by evaluating current protocols and standards of care; providing culturally sensitive and educationally appropriate health management education to patients and families; facilitating consistent, coordinated care and clear communication among all members of the healthcare team through participation in daily interdisciplinary rounds; serving as an advocate for patients and families, identifying and addressing obstacles to care progression and escalating issues as appropriate; serving as a clinical resource for the multidisciplinary team by addressing clinical, process, and system improvement opportunities; participating in organizational and departmental meetings, committees, and workgroups as necessary; demonstrating the highest level of accountability for professional practice and adhering to ethical principles regarding patient confidentiality, informed consent, and adverse outcomes; collaborating with medical staff to support nursing-led protocols; mentoring other APNs, precepting students in APN programs and participating in educational programs for nursing as a clinical expert; identifying patient/family educational needs and collaborating to implement appropriate teaching materials; facilitating and participating in nursing educational programs as a clinical expert; promoting a collaborative work environment supporting the organizational mission, behavior expectations, values, and pillars; promoting innovation by integrating new clinical knowledge into practice; participating in clinical research activities and disseminating findings through practice, education, presentations, and publications; identifying ethical care issues and coordinating interdisciplinary meetings to resolve them; adhering to the legal framework and scope of practice defined by the NJ Division of Consumer Affairs, ensuring compliance with all applicable statutes and professional standards; complying with HMH and department-specific scope of services, policies and procedures as applicable to role; and adhering to HMH Organizational competencies and standards of behavior. Qualifications include a graduate of an NLN/AACN accredited nursing program, a Master's or Doctorate degree from an accredited Nurse Practitioner program, maintaining clinical competencies through continuing education and clinical practice, experience in a relevant patient care setting (or as regulations dictate), a National Provider Identifier (NPI), prescriptive authority through CDS and DEA registration according to New Jersey state law, a collaborative agreement with physicians on staff, excellent written and verbal communication skills, proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms, NJ State Professional Registered Nurse License, Advance Practice Nurse License, BLS AHA Basic Health Care Life Support HCP Certification, Board certified by AANP or ANCC, and AHA Advanced Cardiac Life Support Certification.
