The PT will evaluate residents, develop individualized treatment plans, and provide therapy focused on improving mobility, strength, balance, and overall functional independence.Must-Have Skills: Active California Physical Therapist License Strong patient assessment and treatment planning skills Ability to work in Skilled Nursing / Rehab settings Excellent communication and teamwork skills Accurate clinical documentation skillsExperience Required: Minimum Less than 1 year of PT experience required Skilled Nursing Facility experience preferred New graduates encouraged to apply based on experience levelKey Responsibilities:• Evaluate residents and create individualized treatment plans• Provide therapeutic exercise, mobility, gait, and balance training• Monitor progress and modify treatment plans as needed• Educate residents and caregivers on exercises and safety strategies• Maintain timely and accurate documentation• Collaborate with rehab team and interdisciplinary staff• Promote safe functional independence and quality outcomesCloudinfraIT Inc Job ID #295960.
