Central Nursing - Director of Social ServicesLooking to advance your Social Services career?Skilled nursing of Bensalem is looking to hire for the director of Social Services positionExcellent salary as a Director of Social Services but you must have experienceDirector of Social Services MSW CSW ResponsibilitiesAssists with resident and family transition into the facilityCompletes a comprehensive Psychosocial Assessment for each resident that identifies social, emotional, and psychological needs and strengthsIdentifies medical related social needs of residents, and provides appropriate services to meet the individualCollects and compiles resident social information into personal file upon admissionMaintains lists of facility and community resources available to the residents that can provide assistance in Social Services and clinical carePlans, organizes and implements Social Service programs in the facilityEvaluates effectiveness of Social Service programs in the facility and collaborates the Social Services team, Administration and other facility staff to offer best practices to the residentsAssesses each resident for discharge and coordinates discharges in the facilityAdvocates for resident's best interest and total care in Social Services and clinical careConducts resident interviews and ensures that relevant MDS sections (i.e. cognitive, mood, behavior, patient goal setting) and Care Area Assessments are completed in accordance with regulationPerforms other related job duties as required as a Director of Social ServicesDirector of Social Services Qualifications:Minimum of a Bachelors degree in social work orhuma services related major including sociology, gerontology,psychology, rehab counceling or special ed1 year in healthcare setting where the individual is working as a supervisor social worker who works directly with individualsLong Term Care experienceCompetitive Benefits for a Director of Social ServicesEqual Opportunity Employer
