The RN will ensure quality care, monitor outcomes, and work closely with caregivers, families, and healthcare professionals.Key Responsibilities:Conduct comprehensive in-home assessments and reassessmentsCreate and revise individualized care plans based on clinical findingsProvide medication administration and skilled nursing interventionsMonitor and document changes in client health and statusCommunicate regularly with clients, caregivers, and care teamsInitiate referrals to appropriate healthcare or community resourcesMaintain timely, accurate documentation in accordance with regulationsParticipate and facilitate caregiver in-service programs.
