Care Coordinator

City/State Virginia Beach, VA Work Shift First (Days) Overview Sentara Health Plans is hiring a Care Coordinator for the Tidewater area! This position requires travel for face-to-face assessments on average 3-4 days per week. Days without assessments scheduled employee can work remotely. Applicants must be in Tidewater or Surrounding area. Area includes Virginia Beach, Norfolk, Portsmouth, Newport News, Hampton, Suffolk, Yorktown, Gloucester, and Windsor areas. The Care Coordinator performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate.
